How to obtain your medical records

After 12/15/25:  Send an email to Records@Laptopdoc.net

Important!!!

*  Please specify your First, Middle and Last Name

*  Please specify your date of birth

*  You may be asked to sign a request form that you can download here

*  Records will be in PDF format and burned onto a CD-ROM that we will provide (or a thumb drive that you provide)

*  Records that you personally pick up from us will generally be at no cost

*  Records that you want mailed will generally incur a prepaid charge as set forth by Virginia State Law (§ 8.01-413)

*  Most charts are too lengthy to be faxed
